Clinical Safety Case Report

1. Scope

This report covers the AI Triage Platform as deployed in GP practices in England, from patient entry to the routing outcome shown to practice staff.

2. Intended purpose

The platform routes online consultations to the right level of care. It is not intended for patients under 16 or for people in crisis.

3. Hazard summary

31 hazards were identified. None remains high after controls. Three medium hazards are watched monthly in the Assurance Planner.
